When recurring problems reveal a systems gap

A practical reflection for your next development conversation.

01

Choose a problem that repeatedly interrupts delivery. Describe the observable event, the people involved and the point at which the work becomes unclear. Avoid beginning with blame.

02

Map the handoff: who supplies the input, who decides, who performs the work and who checks the result? Missing ownership or conflicting expectations can make capable people appear ineffective.

03

Agree one change and a review date. Define what improvement would look like and collect simple evidence. Use the review to decide whether the change should continue, change or stop.
